| - If applicant was a widow or widower age 50 to 64 and remarried during the application year
- If renter receives cash public assistance
- If part of homestead was for purposes other than residence, such as operating an in-home business or renting part of your residence

PA-1000 F/G -- 2023 PA Schedule F/G
| - If more than one eligible person is on the deed or lease
- If an applicant is deceased

| - If an applicant is disabled, a physician must complete this form to confirm permanent and total disability
