Use this form to apply for the Property Tax/Rent Rebate Program by mail. Once completed, use these instructions to submit your application. ​

To successfully apply, you may be required to submit additional documents. Use the descriptions below to determine which forms you should include in your application. ​​​

When to Submit​
PA-1000 RC
  • ​If you are a renter ​
PA-1000 A -- 2023 PA Schedule A ​
  • If applicant owned more than one home during the application year 
  • If the applicant has passed away ​
PA-1000 B/D/E
  • If applicant was a widow or widower age 50 to 64 and remarried during the application year  
  • If renter receives cash public assistance 
  • If part of homestead was for purposes other than residence, such as operating an in-home business or renting part of your residence ​
PA-1000 F/G -- 2023 PA Schedule F/G 
  • If more than one eligible person is on the deed or lease 
  • If an applicant is deceased ​
​​PA-1000 PS
  • ​If an applicant is disabled, a physician must complete this form to confirm permanent and total disability  ​
  • ​​If you are submitting on behalf of someone who has died ​​
